Patient Forms

Upon entering our office, my wonderful Chiropractic Assistant (C.A.), Valerie, will greet you and welcome you as a member of our family.  We will request you fill out our patient forms, unless, of course, you already filled out the forms located in the New Patient Center of this website.  The paperwork provides us with general information about yourself and your condition...leading us to the proper approach for the most successful care possible.


Consultation

Next you will have a consultation with Dr. Tyler to discuss your health concerns and health goals, as well as treatment options that interest you.  Many of the sources of your problem(s) will be uncovered in the consultation.  Your first visit is designed for Dr. Tyler to learn more about you, your condition and your expectations to determine whether Chiropractic care can meet your goals and whether or not you are a good candidate for care in our wellness center.


Examination

After your consultation, Dr. Tyler will perform a complete chiropractic examination.  This includes checking your ability to turn and bend in specific directions, performing two computerized nerve scans to determine if you have nerve damage and to what extent, and a bilateral foot assessment to determine how your feet are impacting your spine.  Nothing will be done in our office without your consent.  Our goal is to make you feel very comfortable and confident that our wellness center is the right place for you and your family!


X-Ray Studies

It is a standard routine for all new patients (5 years and older) to have a set of x-rays taken so we may have a thorough evaluation of your spine.  Not only do x-rays help us rule out more serious conditions or assist us in developing the most effective care plan for you, but they act as a "road map."  They provide the specific information necessary to make corrective adjustments so that every patient is able to reach their desired outcome in the shortest time possible and with the least possible expense.  Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.


Report of Findings

Once all the examinations have been performed and all of the information has been gathered, Dr. Tyler will give you a report of findings to answer four questions: 

  1. What is wrong?
  2. Can chiropractic correct your problem?
  3. How long will it take?
  4. How much is it going to cost?

After discussing your history and your goals, examining your spine and reviewing your x-rays and nerve scans, Dr. Tyler will discuss recommendations with you.  If you have a condition that requires care with other providers, the doctor will tell you.  If Dr. Tyler believes we can help you, we will recommend care and a wellness program.


Care

Patients will generally be provided with their first adjustment at the conclusion of their report of findings, if they so choose.  This care is interactive so that you can express any concerns or hesitations you may have.  Everything will be explained thoroughly before you receive care so you have a complete understanding of what will be happening in and to your body. 


Wellness Program

Prior to leaving, patients will be instructed on a wellness program to be conducted at home.  If you are in pain when you first come into our office this may include more specific details, such as ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.  If you desire an overall wellness lifestyle, we will work with you to create good habits and routines that support your success.  Every spine and body is unique and different and therefore, everyone requires a unique and customized wellness plan to fit their needs.  The foundation of our overall wellness lifestyle is maintaining healthy posture and good spinal alignment, eating a healthy diet, keeping your weight under control and managing stress properly.
